Health Promotion Board to unveil HealthTrack SG for chronic disease mgmt
The board has partnered with Google, ConnectedLife, and Fullerton Health for the initiative.
The Singapore Health Promotion Board has partnered with Google, ConnectedLife, and Fullerton Health to launch a pilot programme to support the management of chronic conditions such as hypertension, diabetes mellitus, and hyperlipidemia.
The HealthTrack SG programme integrates clinical data, such as blood pressure, with lifestyle metrics from wearable fitness trackers, consolidating the information onto an online dashboard for doctors.
Doctors can use the dashboard to track the effectiveness of lifestyle modifications and adjust recommendations accordingly.
Meanwhile, patients will also be encouraged to use a mobile application with ConnectedLife to support adherence to their health plans.
