Australia introduces LinkMyCare programme for COPD patients

The initiative will be deployed across 16 general practices over the next 18 months.

Australia is set to introduce the LinkMyCare programme for people with chronic obstructive pulmonary disease (COPD) in South Eastern New South Wales (NSW).

The programme will be implemented across 16 general practices over the next 18 months and will provide general practitioners (GPs) with resources to help patients manage the condition.

The initiative will also aid patients in transitioning between hospitals and GPs to lower the chances of readmission and enhance the information flow between all involved parties. 

The LinkMyCare programme has been co-designed by the South Eastern NSW Strategic Partnership, consisting of the Southern NSW Local Health District (LHD), Illawarra Shoalhaven LHD and COORDINARE - South Eastern NSW Public Health Network.
